If you live within the practice area you can apply to join the practice list.

Reception will ask you to provide your medical card or complete a patient registration form and will also ask for proof of your address such as a utility bill or rent book.   Registration Form and Documentation Required for Registration.

You will also be asked to complete a new questionnaire with information on your health, details of any allergies and medication that you may take. This data will be used  until your medical records are received by the practice.  New Patient Questionnaire

All new patients are asked to arrange an introductory appointment with the practice nurse. The practice nurse will perform a brief medical check and may ask for further details of any health issues provided on your new patient registration form.
